Guide for completing the application for a general practice registrar placement
- GP Registrars are responsible for the timely and correct submission of the Application for a General Practice Registrar Placement.
- Failure to complete the application correctly will result in delays in processing the application at Medicare Australia offices.
- Incomplete applications received at CSQTC will be returned to the GP Registrar.
- GP Registrars are responsible for completing the following sections of the application correctly:
- Registrar Details
- Approved Training Practice Details
- Approved Placement
- Date From and Date To
- the correct dates must be inserted here
- CSQTC will normally only approve application forms for the current training year
- recognition as a general practitioner is limited to attendances provided at the approved practice for the time period (Date From and Date To) specified in the application form
- Practice Name & Street Address
- the correct Practice name, suite number, floor number, street number, street name, suburb and postcode must be inserted here
- the GP Registrar may need to contact the Practice in order to ensure that they have the correct and complete Practice address (eg. if the Practice address has a floor or suite number then this must be included in the address field)
- Telephone Number and Fax Number
- Consortium Name - CSQTC
- Consortium Code - 437
- Full time, Part time, hours per week, Outer Metropolitan Program - tick where appropriate
- Supervisor/Mentor (Trainer) Details
- this section must be completed prior to submission to CSQTC
- when all other sections of the application are complete, the GP Registrar should ensure that the application is sent/given to the Practice with instruction for the Trainer to complete and sign this section and to either return the application to the GP Registrar or directly to CSQTC
- the best way to ensure that the application is completed by the Trainer is to visit the Practice in person
- if the application is posted to the Practice, the GP Regsitrar should keep a copy of the original application
- where the application is left with the Practice, it is the GP Registrar's responsibility to follow up with the practice to ensure that the application has been signed and either returned to the GP Registrar or forwarded on to CSQTC in a timely manner.
- applications received at CSQTC with this section incomplete will be returned to the GP Registrar
- Declaration by the Registrar
- the GP Registrar must read, sign and date this section
